The American Geriatrics Society defines Transitional Care as “a set of actions designed to ensure the coordination and continuity of health care as patients transfer between different locations or different levels of care within the same location." Locations include hospital and nursing setups or home.

Old age comes with its own set of challenges and slow recovery and resilience of health and body are amongst them.
This necessitates rehabilitation before the patient can be comfortably moved to their house.
We provide Transitional Care to the elderly who require rehabilitation either in the form of post-operative care or post icu care.
Post-operative rehabilitation includes care following all major surgeries, where the patient might still need assistance and a relatively lower degree of medical aid.
Post ICU rehabilitation includes care following prolonged ICU Admission for any cause after which the patient may still requires aid and attention before returning home.
We give as much comfort and give a feeling as homely as we can.
However, since this likely to be a temporary arrangement, caretaking expertise has more to do with the nursing and supervision expertise. We provide all necessities for optimum care and satisfaction of the patient. Tube feeding and monitoring of patient parameters like pulse rate and urine output are handled by trained staff.
Additionally, nutritional modifications as guided by our dietician will be done for overall health.
We have 24/7 nursing staff for any iv fluid and oxygen administration or suction/drain that may need to be used for the patient.
